Parent/Guardian Action Required

Please complete the Food Allergy & Anaphylaxis Emergency Care Plan form and Permission to Dispense Medication Form. Then log in to your ePACT account and submit the completed forms.

These forms help ensure we are prepared to safely support your child in the event of an allergy or medical emergency. All required forms must be completed before your child participates in the program.
